Health Center To Do List

Preparing your camper for a stay at an overnight camp can be overwhelming. It is a very exciting time for both campers and parents but the “To Do” list to get ready can feel pretty big!
Falcon has had a packing list since I was a camper in the 80’s and 90’s, if you can believe that! It really does make packing your camper so much easier. I packed myself for years as a camper, then as staff, and I have now been packing myself and my kids for 10 years as a camp nurse. Every year I rely on that packing list!
I’d like the Health Center portion of your camper’s Falcon experience to be just as smooth. So let’s simplify, and make a list!

Health History
The good news is you have already completed the Health History form during the registration process! Step 1 done! This form is in UltraCamp if you would like to review it or update it.
Physician Recommendation Form
The second step is having our physician recommendation form signed by your camper’s medical provider or pediatrician. The provider’s signature on this form gives your camper clearance to participate in activities around camp. This requires a physical within 1 year of the last day your camper is at camp. So if your child had a basketball physical in October chances are very good that your physician will not require a face to face appointment with your camper. Important: If your child takes medications these must be listed on this form. This is the “Physician’s Order” that nurses need to legally administer medications. This includes over the counter items (like vitamins) and prescriptions.
Pre-Camp Nurse Communication
The last step is a pre-camp nurse communication. This is only required if your child takes medications or if there is any significant past medical history that we need to be aware of. This can be an email between the guardian and one of the nurses, or this can be a phone/zoom zoom call. A sign up for a zoom with a camp nurse will be sent out in the spring.
